Who is my contact person? By name, not "our team".
Who writes the code? Often the same person at small service providers – this is fine, but should be known.
What happens in case of illness or vacation? The most important question. Does at least one other person know the project?
Is it outsourced? If parts are assigned to third parties, you should know this – also due to data processing, if data is involved.
Why this is not an impolite question
It is due diligence. A service provider who interprets it as distrust has already answered it.
